For example, see [https://github.com/QubesOS/qubes-issues/issues/1644 #1644]. An issue was reported, nobody has been able to diagnose it, and it fixed itself through an update of upstream software.

And this is not Qubes's fault either. There are too many bugs for it to be humanly possible to track them all down.
